At JD Wellness, we’re committed to creating a comfortable, judgment-free space where your voice is heard and your needs are prioritized.
We block off a full 60 minutes for your initial visit to ensure there's no rush—just quality time focused on you. This first session is an essential step in building a treatment plan tailored to your goals.
đź“„ Intake Forms & Pre-Visit Requirements
- All intake forms will be sent via email and text through Tebra, our secure patient portal.
- These must be completed prior to your appointment—and no less than 2 hours before your scheduled start time.
- The intake packet includes a section for uploading your driver’s license and insurance card. While the form may label this section as “optional,” JD Wellness requires both uploads for telepsychiatry visits.
- If you're using insurance, you'll also need to activate your Headway insurance and billing portal. We’ll send you the setup link via email—please check your spam folder if you don’t see it in your inbox.
- You will receive a text message from JD Wellness via Tebra. This is not spam. Please always read these messages carefully—they often include important links or instructions. You can respond directly to these messages without downloading any additional apps.
đź’» Telepsychiatry Visit Access
- There is no app to download.
- A secure link to join your virtual appointment will be sent via text and email approximately 30 minutes before your scheduled time.
- Simply click the link 5 minutes before your appointment, and your provider will join you shortly.
- If for any reason you do not receive the link or have questions, the best way to reach us is through portal messaging. You can also access this feature directly on our website via the pop-up chat box at jdwellnessllc.com.
What to Expect at Your First Appointment:
- Initial Assessment
- Medical History Review: Your provider will ask about your physical health, past medical issues, and any current medications or supplements you're taking.
- Mental Health Evaluation: We’ll explore your mental health history, including any diagnoses or symptoms such as anxiety, depression, ADHD, and more.
- Medication History: We’ll review medications you've taken in the past—what worked, what didn’t, and any side effects you experienced.
- Discussing Symptoms & Treatment Goals
- Symptom Review: You'll describe what symptoms you’re experiencing, how they impact your daily life, and how long they’ve been present.
- Treatment Goals: We'll talk about what you’re hoping to achieve—whether that’s better focus, mood stability, or improved sleep—so your treatment plan is meaningful and personalized.
- Medication Plan
- Medication Selection: If medication is appropriate, your provider will recommend options and explain how they work.
- Dosing Info: You’ll receive clear instructions on how to take the medication, with guidance on any gradual dose changes.
- Side Effects & Risks: We’ll go over common and serious side effects, so you know what to watch for and when to reach out.
- Treatment Duration: Some medications are short-term; others may be long-term. We’ll explain what to expect and how your progress will be monitored.
- Controlled Substances Policy: If you require controlled substances (such as stimulants or benzodiazepines), you will be scheduled for monthly visits until rapport and treatment stability are established. After that, your provider may recommend visits every 2 to 3 months, depending on your needs and response to treatment.
- Addressing Concerns & Questions
- Open Dialogue: Ask anything—this is your space to bring up concerns or questions about medication, treatment plans, or anything else on your mind.
- Managing Expectations: We'll provide realistic timelines and let you know how we’ll adjust the plan if something isn’t working.
- Monitoring & Follow-Up
- Follow-Up Appointments: Medication management is an ongoing process. Follow-ups help us track progress, adjust medications, and address any side effects.
- Lifestyle & Behavioral Recommendations
- Your provider may suggest complementary strategies like therapy, exercise, mindfulness practices, or sleep hygiene techniques to enhance your care plan.
- Support & Resources
- We’ll offer educational materials and referrals if additional support—like therapy or group counseling—is recommended.

- Be Honest: Share openly about your symptoms, experiences, and any concerns you may have.
- Bring a Medication List: Include all prescriptions, over-the-counter meds, and supplements.
- Note Your Symptoms: Journaling or listing your experiences can help guide your conversation.
- Prepare Questions: Write down anything you’d like to ask—no question is too small.
